You can print most of the account service forms you will need for the Florida College Investment Plan from here, or call Customer Service at 1-800-552-GRAD (4723) and we will mail you the form you need.

Investment Plan Application - You may use this form to open a new Florida College Investment Plan.

Automatic Contribution Form - You may use this form to authorize automatic contributions from your bank account.

Withdrawal Form - You may use this form to request a withdrawal for qualified higher education expenses; a withdrawal for beneficiary (student) death, disability or scholarship; or for a non-qualified withdrawal.

Allocation Change Form - You may use this form to change the allocation of new contributions.

Allocation Transfer Form - You may use this form to transfer money already invested from one investment option to another. Federal law only allows transfers of money already invested once per calendar year or with a change of beneficiary (student).

Beneficiary Change Form - You may use this form to change the beneficiary (student).

Account Owner Change Form - You may use this form to change the account owner.

Cancellation - Rollover Form - You may use this form to request a cancellation or a rollover from the Florida College Investment Plan.

Contribution Coupon - You may use this coupon to contribute to your Investment Plan.

Change of Address Form - You may use this form to change the address of the Account Owner, Survivor and or Beneficiary. The Account Owner must sign in order for the change request to be valid.

The Florida College Investment Plan is not a prepaid plan. Investments in the plan are not insured or guaranteed, and you could lose all or a portion of your investment. Participation in the plan will be sold only by means of a Disclosure Statement and Participation Agreement. A copy of each will be sent to you upon request; you should read them before investing.
