Beneficiary Change Form - You may use this form to change the beneficiary (student).

Account Owner Change Form - You may use this form to change the account owner.

Cancellation Form - You may use this form to cancel your Florida Prepaid College Plan.

Account Owner Change Form (Death) - You may use this form to change the account owner due to the death of the original account owner.

Scholarship Refund Form - You may use this form to request a scholarship refund from the plan.

Transfer Form - You may use this form to request using your Florida College Prepaid Plan at an out-of-state college or private Florida college.

Add Local Fee Plan/Dormitory Plan/Tuition Differential Fee Plan Form - You may use this form to add a Florida Prepaid Local Fee Plan, a Florida Prepaid Dormitory Plan, and /or a Florida Prepaid Tuition Differential Fee Plan for the same beneficiary.

Change of Address Form - You may use this form to change the address of the Account Owner, Survivor and or Beneficiary. The Account Owner must sign in order for the change request to be valid.

Cancellation Form (Death or Disability of Beneficiary) - You may use this form to cancel your Florida Prepaid College Plan due to death or disability of the beneficiary.

Survivor Change Form - You may use this form to change the survivor.
